Forms

To dive with me as a student or just a recreational diver, you will need to fill out a few forms first. Please read and fill them them out completely, leaving nothing blank. You can either e-mail them back to me, or print them out and bring them with you when you come. Please read the sections about liability very thoroughly before making your reservation. If you are not willing to assume and accept responsibility for yourself absolving me of any liability, then you need to find another instructor or guide.

If you are taking a course, you need to fill out all the forms in the student forms box. If you answer yes to any of the questions on the medical questionnaire then you will need to print out the rest of the form, and get approval from a doctor before continuing. 

 

If you are diving with me as a certified diver, then you need to fill out the 3 forms in the certified diver releases box.
